Caldwell University Athletics Medical Team
Caldwell University Athletics has partnered with some of the top medical professionals in New Jersey to provide its student-athletes with excellent medical care. In addition to athletic trainers Heather Arabitg, Ryahn Anson, Richard Lloyd-Young and strength & conditioning specialist David Caposele the Cougars' medical team includes the following medical professionals:
Dr. Michael Kelly, D.O., has been the Cougars' team physician since 2007. He is responsible for coordinating the prevention and care of injuries to Caldwell's student-athletes. Dr. Kelly's sports medicine practice, ProCare Medical Associates LLC in Livingston, N.J., involves the medical care of professional, collegiate and high school athletes. He is a ringside physician for professional boxing and professional mixed martial arts, and serves as the team physician for a number of New Jersey high schools and colleges. An accomplished author, Dr. Kelly has been published in numerous medical journals and he currently is writing his third book on combat sports medicine. He earned his medical degree from the New York College of Osteopathic Medicine and is board certified in Sports Medicine, Internal Medicine, and Ringside Medicine.
JAG Physical Therapy, a comprehensive orthopedic and sports physical therapy center located in West Orange, N.J., serves as the Cougars' physical therapist. JAG PT is New Jersey's leader in rehabilitating foot, ankle, knee, hip, shoulder, elbow, back and related injuries. John Gallucci, Jr., MS, ATC, PT, DPT, the President/Founder of JAG Physical Therapy, has served as an athletic trainer for many entities including: the New York Red Bulls and MetroStars (Major League Soccer), the New Jersey Storm (National Lacrosse League), and the New Jersey Pride (Major League Lacrosse). Gallucci is currently the Medical Consultant to Major League Soccer and has served as a resource to USA Soccer, USA Wrestling, USA Lacrosse, the New York Athletic Club, and the New Jersey Governor's Council on Physical Fitness.
Dr. Frank P. Arena joins the medical team as the team podiatrist. The Director of the Department of Podiatry at Clara Maass Medical Center in Belleville, N.J., Dr. Arena has been in private practice since 1983. He also is the Medical Director of The Wound Care Center at Clara Maass. His memberships include the New Jersey Podiatric Medical Society, the American Podiatric Medical Association and the American Professional Wound Care Association. Dr. Arena is a Diplomate of the American Board of Podiatric Surgery and the American Board of Podiatric Orthopedics and Primary Podiatric Medicine. He also is a Fellow at the American College of Foot and Ankle Surgeons and the American College of Foot and Ankle Orthopedics and Medicine.

Athletic Training Paperwork
All Caldwell University student-athletes must have a physical examination by our team physician before participating in any athletic activities associated with the university's intercollegiate athletics programs. In order to receive a physical, student-athletes must submit completed a Personal Insurance/Pre-participation Physical Form (Link below) at the time of physical. If you have seen a doctor for any new injuries or heart related studies please bring documentation from the doctor. Click on the link below to download the form for printing. Complete the form and bring them on the day of your physical, along with front and back copies of your insurance card(s). You will not be able to fill out this form once you arrive on campus for the physical. NO FORMS, NO PHYSICAL, NO PRACTICE!
